The Scottish Government has guaranteed that post diagnostic support is offered to every person with a dementia diagnosis.
Katie will be following the 8 pillars model of community support and provide post diagnostic support to all who meet this criteria.
Katie will take on this role offering support for one year, following a dementia diagnosis. Providing a coordinated approach to support individuals adjusting to and managing the impact of early stage dementia.
This will include advising on personalised support, creating community connections and ensuring general and mental health care treatment is sufficient.
This work should provide people living with dementia and also their carers/families with the tools they will require for their dementia journey.
Katie is now based at the Community Mental Health Team (older adults) at Falkirk Community Hospital and works in liaison with community psychiatric nurses and healthcare professionals withing the CMHT.
