We are aiming to ensure people with a dementia diagnosis can access services at the right time. Relationships of trust are developed which enables continuity for all throughout their dementia journey.
We have provided an environment which is trauma informed and dementia friendly which enables everyone to feel safe and accepted.
We provide sessions on 3 evenings and 3 sessions on a Saturday. Up to 5 people can attend each session. This gives the service users the opportunity for social interaction on a 1:1 basis with others who attend the sessions, and to have the opportunity to engage and participate in activities which meet their needs. We also offer support to family members and maintain regular contact.
This service has proved invaluable for both service users and families and we have been informed of this verbally. As most service users have no other services in place we are the one consistent support in their lives. Although this is still a new service, we have noted positive changes in physical and cognitive health, and families have informed us of the positive impact receiving respite on an evening or Saturday has had for them. We can assess and monitor each person, and pass on any important information to other professionals as we are continually working to prevent a crisis situation occurring.
